Submit a letter addressed to "Certificate of Good Standing" at any of these addresses, requesting a certificate and including a check or money order payable to the Supreme Court in the amount of $25.00.  Cash is acceptable if hand-delivered.  Include a self-addressed envelope.  The processing fee is non-refundable.  Requests for certificates must include the attorney's full name, identification number and be signed by the attorney.
